1.0 IMPORTANT INFORMATION!!

You are expected to register your institution details by filling in registration form.All the fields are mandatory and Thirdparties are required to fill them.
You are required to attach the following documents
1.  Registration Certficate.
2.  Memorandum and Articles of Association.
3.  Current Business License.
4.  Letter requesting authority to use the check-off/salary remittance.
5.  letter Confirming membership with your Association.
6.  List of Clients from The Civil Service,Teachers Service Commission,Department of Defence and state corporation.
7.  Board of Directors/AGM meeting resolution.
8.  Business Income Tax Personal Identification Number(PIN).
9.  Current City Council Single Business Permit.
10.  For SACCOS offering FOSA to attach authority letter from commissioner of cooperatives.
11.  Welfare Association and Self help Group to attach duly signed Constitution.
NB:You cant complete registration without attaching the above documents
THIRDPARTY SERVICES WILL ATTRACT FOLLOWING CHARGES
1.  One time Registration fee .
2.  Annual Renewal Registration.
3.  Creditworthness verification per request .
4.  Monthly deduction processing fee.
